I don't want to live with her anymore

My mom doesn't do anything unless I beg her and sometimes she doesn't even do it then. I've been having this pain in my back for about four years now and hasn't taken me to do the doctor until this year but still nothing has been done. She hasn't called them back so we could make the appointment for an X-ray and the pain is just getting worse and I know she sees that everyday. I told her I wanted to start working so I needed to her to fill out these papers so I could get my working papers she's had the paperwork sitting on her dresser almost for the whole school year. So I told her I had to go back to school for my report card a couple of weeks back and she said she'd try to get an appointment with my doctor so he could fill out the paperwork but she never even tried to make an appointment no call or nothing.
